Tubal Reversal

Introduction

Tubal reversal, also known as microsurgical tubal reanastomosis, is a procedure performed on women who have had their tubes “tied” and is their only cause for infertility.

If you have had a tubal ligation, we can help you decide if reversal is right for you based on your medical records. If you do not qualify for this procedure due to complete absence of tubes or low sperm count, you may still inquire about In Vitro Fertilization (IVF) at our Center.

Success Rates

Dr Vermesh has vast experience in the performance of reproductive surgery including tubal reversal. As a Board Certified Reproductive Surgeon, he has taught the tubal reversal procedure to fellows at the University of Southern California and has performed thousands of procedures since 1985. The success rate depends on a variety of factors including woman′s age, condition of her fallopian tubes, sperm quality and others. In almost every case, one or both tubes are successfully reconnected as evidenced by a dye test during the operation. If pregnancy does not occur within 6 months, an X-Ray study (HSG) will show if the tubes are still open. Conception rate following tubal reversal at our Center is approximately 70% within one year.

Tubal Reversal Procedure

The procedure is performed in an outpatient facility under general anesthesia through a small incision below the pubic hair line (bikini). Dr Vermesh uses microsurgical techniques involving special instruments and hair thin sutures as well as special optics for magnification. The length of the procedure is about 1-2 hours. The skill and experience of the surgeon are very important to the overall success. Upon entering the pelvic cavity, the uterus and tubes are inspected. Typically there are two remaining tubal segments, the proximal segment that emerges from the uterus, and the distal segment that ends with the fimbria next to the ovary. The total length of the proximal and distal segments is measured to ensure that enough tube is available. After opening the blocked ends of the remaining tubal segments, Dr Vermesh evaluates their patency by injecting blue colored fluid through the vagina and by passing a stent. The reopened tubal ends are then reconnected. Injection of blue colored fluid through the vagina confirms that the tube is open. The small abdominal incision is then closed with absorbable sutures and injected with long lasting local anesthesia.

Recovery And Post-Operative Period

You will awake from the procedure in the recovery room, where you will be observed for 2-3 hours. Upon discharge, you will receive a prescription for pain medication and instructions regarding diet and activity. In general we recommend return to normal diet within 1-2 days and return to normal activity within 7 days. Wound dressing may be removed after 2 days.

You can try to conceive one month after the procedure. If you are not pregnant after 6 months of trying, Dr Vermesh will recommend an x-ray of your uterus and tubes (HSG). If your tubes are blocked you will need to proceed with In Vitro Fertilization (IVF). If your tubes are open, we will advise you on management options.
